Remote patient monitoring, also known as RPM, uses technology to monitor a patient’s condition from their own home.

Specific devices can be prescribed through your provider to monitor chronic medical conditions such as diabetes, obesity, heart failure, high blood pressure, asthma, COPD, as well as many others.

These devices allow you to monitor your readings at home and have this data uploaded to a portal for your provider to view in real-time. If your readings are not where they should be, your provider can meet with you so changes can be made sooner.
